Please mail back to us at the address below at least 1 week prior to your appointment or if time doesn’t allow mailing, please fax to 912-427-6029. If you are unable to get the completed forms back to us prior to the appointment, please call our office at (912) 427-0800, so we can help you make the proper adjustments. Our goal for you is to minimize your waiting room time.
Please read the Notice of Privacy Practices available below. When you present for your appointment you will be asked if you understand the Privacy issues and sign accordingly. You do not need to mail in or complete the Notice of Privacy Practices.
If you have insurance, be prepared to pay your co-pay and any deductible.
Please bring with you a pair of comfortable shorts to put on during your exam.
